156 14-Apr health attendant from the SGH urology centre 26-Apr 54 F 129 a 54 year old woman who was a health attendant at SGH Urology Centre. She had been admitted to TTSH since 9 April 2003.

Four staff from SGH Urology Outpatient Centre and a visitor who was at the SGH Diagnostic Radiology Barium Investigation corridor were admitted to TTSH over the weekend. These two areas are adjacent and connected to each other. The visitor and a staff are probable cases. The other 3 staff are suspects. Two of the urology staff are healthcare assistants in the Urology Outpatient Centre. One of them, a new employee, started work on 1 April 03 and became symptomatic on 4 April 03. The other was her supervisor and she became ill on 7 April 03. The other 2 urology staff are staff nurses.

159 15-Apr Staff-15/4         There are 4 additional SARS cases reported today, all linked to the SGH cluster. These are patients who had been earlier admitted to TTSH as suspect cases and who have now been diagnosed as having SARS as they have developed signs of lung infection on chest-X-ray. They comprise of 1 SGH staff, 1 inpatient and 2 visitors. There was a SARS patient in intensive care who died today. He was a relative of one of the three initial index cases.

The doctor attended to a patient who was being treated for heart failure. The patient, who was admitted to NUH on 8 April, had been a visitor at SGH Ward 57 in end March. He had multiple medical conditions and did not have fever at that time. When he was reviewed in the ward, the possibility of concurrent pneumonia was recognised as his condition had deteriorated. The patient was unstable and required transfer to the NUH Intensive Care Unit where he was intubated. The transfer to TTSH took place later on the same day when the patient's condition stabilised. The patient had since passed away on 12 April 2003.

The NUH doctor developed fever on 11 April 2003 evening and was sent to TTSH Emergency Department on the same day. She has been diagnosed as a probable SARS case today, 16 April 2003.

He works as a taxi driver. On 10 April 03, he consulted a GP after working from 6 am to 9 am. He remained at home for the rest of that day and the next. On 12 April 03, he consulted a GP again. As he was still unwell, he was admitted to TTSH that afternoon.

176 19-Apr Cab Driver   54 M 130 54-year-old taxi driver who ferried [130] from his home to PPWM on 5 Apr 03. He became unwell on 14 April 03 and was admitted to TTSH on 18 April 03.

198 27-Apr Unlucky's Husband   33 M 178 a 33-year-old man who is the husband of the 28-year-old woman announced as a probable SARS case on 20 April. He had been admitted to TTSH as a suspect case on 23 April 3.
199 27-Apr Dr Alexandre Chao 22-Apr 37 M 116 Today, there is only one SARS case reported. The patient was Dr Alexandre Chao who passed away on 22nd April. Post mortem results confirmed his case as SARS.

Dr Alexandre Chao, Consultant Vascular Surgeon with the Department of General Surgery, SGH was on annual leave until 7 April 03 when he returned to SGH to work

He became febrile on the evening of 12 April 03 (37.4 degree C) and had isolated himself from his family in another house. The following day, 13 April 03, he became febrile again and visited the SGH Staff Clinic and was given 2 days medical leave.

On 15 April 03, he still had a temperature of 37.7 degree C and was admitted to an isolation room in SGH Ward 47, our Isolation Ward. He had no chest complaints. The clinical diagnosis was dengue fever.

On 16 April 03, he had developed symptoms which were consistent with that of dengue fever, including rashes over his body, back and limbs.

Blood tests done on 17 April 03 showed that his platelets were slightly low and further depressed on 18 April 03. This had returned to normal by 21 April 03.His chest x-ray taken on 17 April 03 was normal. On 19 April 03, the x-ray film did not show significant abnormalities. He remained afebrile on 21 April 03. Dr Chao had reported that he was feeling better in terms of his fever and myalgia, although he later complained of a sore throat.

On the morning of 22 April 03, Dr Chao had complained of a sudden onset of breathlessness. He was transferred to the intensive care unit and was intubated for respiratory distress. Chest x-ray done prior to intubation showed bilateral pneumonia. Despite intense treatment, he passed away at about 7.00 pm. A post-mortem was carried out on 23 April 03. Testing of postmortem tissues subsequently confirmed SARS.

Dr Chao may have been infected by a critically ill patient from the Ward 57 & 58 cohort, whom he cared for during the week of 7 to 13 April 03. This patient collapsed on 6 April 03 when he was supposed to be transferred to TTSH. In view of the patient's critical condition, he was therefore isolated in the SGH Neurology ICU until his condition was stable for transfer on 14 April 03. He was subsequently classified as a probable SARS case at TTSH and passed away on 25 April 03.
200 29-Apr 37yo on quarantine   37 M 196 One is the 37 year old brother of the probable SARS case (the 41-year old man) announced on 26 Apr 03. He has been on home quarantine since the 20th April. He become unwell on 27 April 03 and was admitted to TTSH 28 April 03.
201 29-Apr Another Taxi Driver 19-Apr 43 M Unknown The other new case is a 43 year old man who worked as a taxidriver. He developed a fever on Friday, 11 April 03. From the 12 April 02, he did not return to work and remained at home. He consulted his GP on 12 April 03 and again on 15 April 03 when he was referred to the Emergency Department (ED) of Changi General Hospital for viral fever. At CGH ED, blood tests were done which were normal. He was diagnosed as having viral fever and discharged with two days of medical leave. On the morning of 19 April 03, he complained of sudden onset of breathlessness and collapsed at home. He was brought by ambulance to CGH. Despite efforts to resuscitate him, he was pronounced dead at about 9.30 am on 19 April 03. As he was classified as a Coroner's case, a post mortem was conducted on 20 April 03. Microscopic examination of the tissues showed lung damage from a possible infection. Tissues taken for further investigations have been reported as being positive for coronavirus. Investigations are underway to determine the source of his infection and contact tracing is in progress.
